About the service

We offer physical health and wellbeing checks for people who have a serious mental illness.

We aim to reduce the risk of people becoming physically unwell from illnesses such as cardiovascular diseaseĀ (CVD), endocrine disease, such as diabetes, and respiratory (breathing) problems.

We have developed a monitoring tool so that we can carry out a thorough check of physical health and wellbeing.

The types of checks that our associate practitioner offer are:

  • Blood pressure
  • Temperature
  • Pulse
  • Blood test
  • ECG (rhythm and strength of your heart)
  • Weight
  • Height
  • Lifestyle discussions around smoking and diet

This service is for adults living in Bradford, Airedale, Wharfedale and Craven who are prescribed anti-psychotic medication.


How to access

We accept referrals from consultants, care co-ordinators and in some case GPs.
