About the service

We understand how complicated it can be when you have a range of complex health and social care needs. We will help you and your family to manage your long-term health needs and treatment more effectively.

We will help to co-ordinate and plan your health care needs and attempt to reduce preventable admissions to hospital. We will undertake an assessment focusing on your health care history and develop a care plan based on your individual requirements.

If you require a specialist service, we can make relevant referrals. We can make sure your views are heard and support you to remain independent, and in your own home, as long as possible, if that is your wish.

How to access

To receive the help and support of our case managers you can self-refer, this means that you, or your family, can contact the team direct 01274 256131.

Your GP or any other health and social care professional may suggest that our case managers can help you. In this case they can make a referral on your behalf.
