About the service

Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families living with all types of dementia.

Admiral Nurses are qualified registered nurses with significant experience of working with people living with dementia before becoming an Admiral Nurse. They are part of our multi-disciplinary Proactive Care Team (PACT) working within specific communities.

To help you to cope with dementia, live more positively and face challenges with more confidence and less fear, Admiral Nurses offer one-to-one support, particularly during challenging or difficult times. They listen to really understand, so they can answer your questions and share techniques. For example, they can help you to prevent or manage the fear or distress that someone with dementia can experience; stay connected if communication gets hard, and cope if you’re struggling.

How to access

Our Admiral Nursing service forms part of our multi-disciplinary Proactive Care Team (PACT), which operates within specific areas of central Bradford.

If you are caring for someone who has dementia and they, or you, are registered with one of these GP practices in central Bradford, please speak to your GP who can refer you to the Proactive Care Team’s Admiral Nursing service. You can also self-refer via the Single Point of Access number 01274 256 131.

If you are caring for someone with dementia who does not live in central Bradford and is not registered with one of the GP practices linked to above, please call the Admiral Nurse national dementia helpline for advice, or contact Dementia UK, or speak to your GP.

How to find us

While we have a base at the Horton Park Centre, we conduct most of our work in people’s houses on home visits. We can meet you in a public place if you would feel more comfortable or if needed we can see you in a clinic.

What to expect

Referral

When you are referred to the Bradford Admiral Nurses one of the team will usually call you to discuss the referral to help us decide if we are the best team to support you, or if there are other services that better suit your needs. We can support with an onward referral if necessary. At this point if we believe you would benefit from our service we will usually arrange an initial assessment.

Waiting list

We aim to contact you within two weeks of referral. There may be a time when this is not possible and we will discuss this with you during our initial phone call. If your GP is not currently in our catchment area, we will try to give you an initial phone call to discuss any support you need and direct you to the most appropriate help. This could be your GP or other services for people with dementia and their carers.

Assessment

If you or the person you care for have a confirmed diagnosis of dementia and are registered at one of our GP practices and we feel at our initial phone call that we are the right service for you, we will arrange an initial assessment. At this assessment we will usually focus on the person with dementia and ask questions about their physical, mental and emotional health and identify any areas that we can work on to improve their quality of life. At further appointments we will offer the same assessment to carers to focus on their needs.

After your assessment

There is no set time frame for people who are supported by Admiral Nurses. Some people get what they need from one or two visits, other families may be supported for a much longer time. This all depends on what sort of help and support you need and any other changes along the way. We accept re-referrals for people who have used our service in the past and acceptance is based on the level of need.

For professionals

Referral criteria

The person with dementia must have a confirmed diagnosis of a dementia, made either by a Memory Service or Consultant.

The person with dementia is experiencing complex symptoms and carers are strugging to cope and would benefit from specialist, focused support.

The person with dementia or their carer has consented to the referral and receipt of the service.

Exclusions

The primary need of the person with dementia, carer or family is not related to the dementia (for example a significant mental health problem requiring a different service, or underlying physiological problems which could be causing neurological impairment)

Head injury that is a primary diagnosis to cognitive impairment.

Crisis management requiring response within 120 hours.
